Introduction: Evaluation of facial change during normal/abnormal function is important for many health care professionals such as orthodontists, maxillo-facial surgeons and speech therapists. Aims: To evaluate the reliability in assessing speech and to assess dynamic change as a result of surgical intervention. Subjects and Methods: Reliability: 22 subjects were asked to phonate the word " puppy " in a normal, relaxed manner twice in a 10 second sequence. The sequence was recorded using a non-invasive, three-dimensional motion analysis image capture system (3DMDface™ Dynamic System) at 48 frames per second. Match sequenced shells (for the word " Puppy " were superimposed and colour deviation maps were analysed to assess differences. Application: The 10 second sequence was recorded prior to surgery, one month and three months after surgery (for an individual requiring advancement of the maxilla and setback of the mandible). The images were referenced to fixed X, Y, Z planes and the change in lip movement was evaluated. Results: The mean facial shell difference for matched sequences was 86.2% range 66.8-97.5%). The surgical application showed significant unpredictable change. Conclusion: The method to assess speech was considered very reliable. The motion capture system seems an appropriate method to assess soft tissue change as a result of surgical intervention.
